When a problem related to health care quality and patient safety arises, there can be an

inclination to focus on a specific event rather than to examine the larger context

surrounding it. Analyzing the systems and structures within an organization provides a

foundation for generating a fuller understanding of how and why the event occurred and

for developing strategies to address the underlying issues. Perhaps more importantly,

such an analysis can be used to proactively identify potential challenges and improve

organizational systems and structures in order to promote positive outcomes.

In the first section of your Course Project, you analyze the systems and structures of a

health care organization with which you are familiar.

To prepare:

Identify a health care organization with which you are familiar; one that you will be

able to sufficiently analyze to complete the full scope of the Course Project. (Refer to

the Course Project Overview as needed.)

Select two or more of the following frameworks:

· Learning organizations

· Complex adaptive systems (CAS)

· Clinical microsystems

· Good to great

· The 5 Ps

With these frameworks in mind, analyze the systems and structures of the

organization. Be sure to research and consider:

· Mission, vision, and values

· Strategic plan, goals, and objectives, if possible

· Key operational processes and patterns

· Organizational priorities and investments, as indicated by financial data

Week 4: Organizational Systems and Structures, Part 2

Section 1: Organizational Systems and Structures Evaluation

As you continue your evaluation of a health care organization, it is essential to pay

attention to culture. Organizational culture provides the context in which all interactions

and processes occur and is therefore central to any effort to enact change.

To prepare:

Continue to analyze your selected organization, as indicated in Week 3 (Section 1 of

the Course Project).

Continue to analyze essential elements of organizational culture and evaluate the

influence of culture on the ability to achieve goals within your selected organization.

To complete:

Write a 3- to 5-page paper in which you:

· Provide a description of the organization that you selected

· Present your analysis of the organization with attention to:

· Its mission, vision, values

· Strategic plan, goals, and objectives

· Key operational processes and patterns

To help you move forward with your Course Project, this week you identify a quality

issue that you would like to address and submit a description of this issue for Instructor

feedback.

Your proposed issue should reflect a genuine need for improvement within your

selected health care organization. Through your Course Project, you will analyze this 

issue and propose strategies to improve outcome(s), including the redesign of a related

process.

To prepare:

· Review the six quality improvement aims from the Institute of Medicine (IOM)

· Safe: Avoiding harm to patients from the care that is intended to help them.

· Effective: Providing services based on scientific knowledge to all who could benefit and refraining from providing services to those not likely to benefit (avoiding underuse and misuse, respectively).

· Patient-centered: Providing care that is respectful of and responsive to individual patient preferences, needs, and values and ensuring that patient values guide all clinical decisions.

· Timely: Reducing waits and sometimes harmful delays for both those who receive and those who give care.

Diabetic Ketoacidosis (DKA) is a very serious and common complication of type 1 diabetes, but can also occur in individuals with type 2 diabetes. According to Misra and Oliver (2015) DKA has a mortality rate of 3-5%, and 6% of DKA cases are the initial presentation of diabetes in an individual.

DKA most often occurs due to an illness, such as an infection, “when there is an absolute or relative deficiency of insulin and an increase in the levels of insulin counterregulatory hormones” (McCance et al., 2013, p. 744). The increase of these hormones, along with the deficiency of insulin, cause the body to go into a state in which there is “decreased glucose uptake, increased fat mobilization with release of fatty acids, and accelerated gluconeogenesis and ketogenesis” (McCance et al., p. 744). Additionally, increased ketone concentrations are caused, which then causes a loss of bicarbonate, and leads to the development of a metabolic acidosis.

Hallmark signs of DKA include polyuria, polydipsia, lethargy, nausea, vomiting, altered mental status, Kussmaul respirations, and sweet-smelling (acetone) breath.

In the case of Ms. Blake, the following are all abnormal lab results: Sodium 156; Chloride 115; ABG pH 7.30, PO2 70 and HCO3 20. The ABG values show a metabolic acidosis as well as a low blood oxygen saturation. The sodium may be elevated in this case due to severe dehydration, which can also cause other dangerous electrolyte abnormalities.

For patients with all types of diabetes, it is important to ensure that they understand the many complications that are associated with their diagnosis. For Ms. Blake, it is important to ensure that she understands the severity of her DKA diagnosis, and how it may have been prevented. Education should also be reinforced in regard to the home monitoring of glucose and ketone levels. Additionally, it is important to find out if she has family or friends that are available to check in with her frequently, especially when she if feeling unwell, so that she can be seen by a physician sooner rather than later.

POST 2

Week five case study involves a three-month-old infant diagnosed with pyloric stenosis. The etiology behind pyloric stenosis is unknown at this time but it is thought to have a connection to genetics and environmental factors.  The pathophysiology behind pyloric stenosis involved a thickening of antropyloric portion of the stomach which the lumen has edematous mucosa which cause an obstruction (Hernanz-Schulman, 2018). The hyperplasia and hypertrophy of the muscle literally blocks of the pylorus which inhibits the passage of food to be digested.  The gastric outlet obstruction then causes food to not be able to pass and thus leads to forcefully vomiting.  This typically occurs in infants three to five weeks old (Hernanz-Schulman, 2018).  The classic signs that are hallmark include vomiting, persistent hunger, changes in bowel movements, dehydration, and decreased weight (Hernanz-Schulman, 2018). This case study described that the infant was having frequent episodes of vomiting after eating, swollen belly, fussiness due to being hungry, and decreased weight because the body was not being able absorb any food because it was being blocked from small intestine. 

                Complications from pyloric stenosis include failure to grow, jaundice due to bilirubin buildup, stomach irritation and dehydration (Hernanz-Schulman, 2018).  On assessment a provider may be able to palpate and fill an olive like lump that indicates an enlarge pylorus. They may order follow up ultrasound testing as well. The only treatment for pyloric stenosis is surgical intervention. The surgical intervention known as pyloromyotomy will help open up the pylorus enable food to be able to pass through. Post pyloromyotomy the patient will be able to have gastric emptying which will allow for proper digestion. It would be important to teach the parents on what exactly is happening with the anatomy of the child and why they are presenting with these signs and symptoms. The provider will need to educate them that they will need to immediately see a gastroenterologist or pediatrician for immediate surgery to fix this problem. It is important to let them know these procedures can be done laparoscopic ally and is minimally invasive through the naval.  Once the procedure is completed the provider should let the family know they may still have some vomiting post opt and will be able to eat normally within twenty-four hours. Additionally, it is important to educate them on fluids post opt.  The main thing to stress to these parents is how essential surgical intervention is so the child can eat, grow, and absorb nutrient adequately.

Your supervisor has asked you to do a 15-minute oral presentation at a staff meeting about a recent issue that occurred at another hospital in town. Following an industrial accident, two patients arrived at the emergency room of that hospital at the same time, presenting with very similar inhalation injuries. The hospital received a great deal of negative press due to how the patients were triaged in the ER. Your supervisor would like you to use the specifics of this case to review triage procedures and best practices at your facility. Here are the details:

  • One is a 32-year-old firefighter, Frank Jeffers, who is presenting with respiratory difficulties that he obtained while evacuating victims of an industrial accident. He is a married homeowner and father of two young boys. He has lived in the community all his life. He has full and comprehensive health insurance through his employer.
  • The other is Brent Damascus, a 58-year-old man. Brent is presenting with respiratory difficulties with the same intensity as Mr. Jeffers above. He is well known at the hospital emergency room, as he is a frequent visitor with various complaints, including asthma, headaches, and tremors. He is homeless, unemployed, and uninsured. He stays many nights at the YMCA and eats lunch at the soup kitchen. He has lived in the community for over 10 years and has been arrested several times for petty theft.

Preparation

Search the Capella library and the Internet for scholarly and professional peer-reviewed articles on best practices in triage nursing. You will need at least three articles to use as support for your work on this assessment.

Directions

Create a 15-minute presentation (3–4 pages) that examines the moral and ethical issues that occurred when triaging these two patients and the best practices for managing this in the future.

Divide your draft into a number of talking points that you can summarize neatly. Keep in mind that an oral presentation requires slightly different language than an essay. The aim is to communicate your message so keep sentences simple and focus on the key points you want to deliver. Address the following in your presentation:

            My experience with using Grammarly and Safe Assign were not as daunting as I expected.  Grammarly is an app-based program that can be attached to Office 365 and Firefox.  The link for Grammarly was found on the academic guides page.  I used the site search box to find the app’s location. From there it was as easy as following directions and downloading the app.  I copy and pasted the paraphrased assignment and received an 83/100 for performance.  Other scores given were for word and sentence length, those scores were a 4.9 and a10.9.  The only issue found was the “Go Premium” offer that was $11.99 a month or $139.95a year.  There will not be a subscription ordered.  Grammarly will be used in my Word program on a trial basis. It can’t hurt to see what needs to be fixed in real time.  Another academic integrity tool is Safe Assign.  This tool is defined in the academic answers portion of the academic guide by stating, “SafeAssign is a tool that helps students prevent plagiarism and aids in identifying opportunities to add properly cited sources rather than just paraphrasing.” (Walden, 2011) Finding Safe Assign in Blackboard was very easy.  It is in the toolbar on the bottom left-hand side of the page.  Once on the site, it is as easy as browsing your computer for the document and simply upload it.   The only issue I have with Safe Assign is it is not in real time, you must wait for the site to review your work then make the changes needed.  In my opinion, Grammarly is a better tool for writing and having a real-time view of the paper being written.

A resource that I would recommend is the Online Writing Lab (OWL).  It is the base from where the American Psychological Association (APA) is explained in detail.  In the APA overview and workshop section of the OWL, it states, “Establish your credibility or ethos in the field by demonstrating an awareness of your audience and their needs as fellow researchers.” (Purdue Writing Lab, 2018) This is why we should use APA when scholarly writing.  Credibility is key when writing anything at a master’s level.  When using APA, we research ideas and information from scientific journals, and at the same time distribute that knowledge in the form of scholarly writings.  Using the OWL for any APA formatting question creates a solid base for building an educational publication to be reviewed by your peers. (APA, 2010, pg.9)

Grammar in its basic form decides how a writer will be received by the reader.  Incorrect grammar sidetracks the reader from the tone an author has set.  A format that I constantly work on is the use of parallel construction.  On page 84 of, the “Publication Manual of the American Psychological Association (6th ed.)” parallel construction is covered.  This is two ideas covered in a sentence that will improve the reader's comprehension.  (APA, 2010, pg.84) Another format important to consider is the explanation of abbreviations.  Abbreviations in scientific writing are used to shorten long drawn out titles. The key in using abbreviations is to initially explain what the abbreviation is and use it at least four times.  There is a balance that must be attained.  Using too many abbreviations jumbles up the sentence and make it difficult to read, but not using enough causes the reader to lose interest with repetitive phrasing. Discussion: Assessing the Ears, Nose, and Throat

Most ear, nose, and throat conditions that arise in non-critical care settings are minor in nature. However, subtle symptoms can sometimes escalate into life-threatening conditions that require prompt assessment and treatment. Nurses conducting assessments of the ears, nose, and throat must be able to identify the small differences between life-threatening conditions and benign ones. For instance, if a patient with a sore throat and a runny nose also has inflamed lymph nodes, the inflammation is probably due to the pathogen causing the sore throat rather than a case of throat cancer. With this knowledge and a sufficient patient health history, a nurse would not need to escalate the assessment to a biopsy or an MRI of the lymph nodes, but would probably perform a simple strep test.

In this Discussion, you consider case studies of abnormal findings from patients in a clinical setting. You determine what history should be collected from the patients, what physical exams and diagnostic tests should be conducted, and formulate a differential diagnosis with several possible conditions.

Note: By Day 1 of this week, your Instructor will have assigned you to one of the following specific case studies for this Discussion. Also, your Discussion post should be in the Episodic/Focused SOAP Note format, rather than the traditional narrative style Discussion posting format. Case 1: Nose Focused Exam
Richard is a 50-year-old male with nasal congestion, sneezing, rhinorrhea, and postnasal drainage. Richard has struggled with an itchy nose, eyes, palate, and ears for 5 days. As you check his ears and throat for redness and inflammation, you notice him touch his fingers to the bridge of his nose to press and rub there. He says he's taken Mucinex OTC the past two nights to help him breathe while he sleeps. When you ask if the Mucinex has helped at all, he sneers slightly and gestures that the improvement is only minimal. Richard is alert and oriented. He has pale, boggy nasal mucosa with clear thin secretions and enlarged nasal turbinates, which obstruct airway flow but his lungs are clear. His tonsils are not enlarged but his throat is mildly erythematous.

Case 2: Focused Throat Exam
Lily is a 20-year-old student at the local community college. When some of her friends and classmates told her about an outbreak of flu-like symptoms sweeping her campus over the past two weeks, Lily figured she shouldn't take her three-day sore throat lightly. Your clinic has treated a few cases similar to Lily's. All the patients reported decreased appetite, headaches, and pain with swallowing. As Lily recounts these symptoms to you, you notice that she has a runny nose and a slight hoarseness in her voice but doesn't sound congested.

Case 3: Focused Ear Exam
Martha brings her 11-year old grandson, James, to your clinic to have his right ear checked. He has complained to her about a mild earache for the past two days. His grandmother believes that he feels warm but did not verify this with a thermometer. James states that the pain was worse while he was falling asleep and that it was harder for him to hear. When you begin basic assessments, you notice that James has a prominent tan. When you ask him how he's been spending his summer, James responds that he's been spending a lot of time in the pool.

To prepare:

With regard to the case study you were assigned:

· Review this week's Learning Resources and consider the insights they provide.

· Consider what history would be necessary to collect from the patient.

· Consider what physical exams and diagnostic tests would be appropriate to gather more information about the patient's condition. How would the results be used to make a diagnosis?

The city in which you work has undergone a significant population shift within the last 10 years, due to increased employment, education opportunities, and an influx of immigrants and refugees. To put the population shift in perspective, the local school district enrollment report states that nearly 100 languages are spoken in the schools, up from 32 languages in the last census. Senior leadership has asked you to examine all types of multiculturalism and diversitywithin your health care organization.
To do this, you will need to use a systems perspective to examine the relationship between a diverse workforce and patient outcomes; identify gaps between best practices and the organization's diversity policies, and make a recommendation for organizational change.

In the executive summary:

This week's topic  is :  The Case of the Sweaty Salesman

Michael is a 30 year old salesman, who spends approximately 4 days each week traveling to visit with customers in his region. During his routine physical, he casually mentions to his physician that he seems to be sweating more profusely than normal and mentions that most rooms that once felt comfortable are now too "hot". He thought that it was simply due to the change in seasons and companies being slow to turn down the thermostats from the winter temperatures, however, this problem seems to persist even when he is at home. A room that his wife and children find to be comfortable causes him to sweat profusely. Michael also reports that he seems to be loosing weight even though his appetite has increased. He also complains that he has a shortened attention span and that he always wants to be moving around. Despite the fact that he feels fatigued, Michael claims to have difficulty sleeping and seems to have more frequent bowel movements, occasionally accompanied by diarrhea. The physician checks Michael's medical history and finds that, indeed, he has lost 15 pounds since his last physical. Wanting to rule out the possibility of infection with the human immunodeficiency virus (HIV), the physician asks Michael questions about his sexual history and practices and finds that he is not at risk for HIV infection. Also, in checking his chart, the physician finds that Michael has a negative history for chronic illnesses, does not smoke, and has a low risk for cardiovascular disease. He does, however, have a positive family history for autoimmune diseases. His father suffers from idiopathic thrombocytopenia, his mother has been diagnosed with rheumatoid arthritis and his oldest sister was recently diagnosed with systemic lupus erythematosus.

Results of Michael's physical examination were within normal ranges with the exception of the following: he demonstrated tachycardia, loud heart sounds, and apparent cardiac arrhythmias accompanied by slight hypertension. These arrhythmias were confirmed by electrocardiogram to be supraventricular in origin. Michael's eyeballs appeared large and protruding and his hair was fine and soft. He was also beginning to demonstrate some degree of alopecia. Michael was also observed to have palmar erythema. Palpation of the neck revealed the presence of goiter. Results of blood tests indicated elevated concentrations of thyroid hormones (thyroxin and triiodothyronine), hypercalcemia, and decreased circulating concentrations of lipids. Based on the physical characteristics and the results of the blood tests, Michael's physician suspected that Michael was suffering from some form of hyperthyroidism and sent him to an endocrinologist to confirm the initial diagnosis.

Following the initial consultation and examination, the endocrinologist ordered tests to determine whether Michael has a hyperthyroid. The test results indicated an elevation in the concentration of thyroid hormones in the blood and the presence of thyroid-stimulating antibodies. These antibodies specifically stimulate the thyroid gland by binding with the thyroid stimulating hormone receptor located on the plasma membrane of the follicular cells of the thyroid gland. Based on these results, the endocrinologist concluded that Michael has Grave's disease, a form of hyperthyroidism believed to be autoimmune in nature. Michael was presented with a number of possible treatment options. These included treatment with chemicals (propylthiouracil and methimazole) that decrease the production of thyroid hormones by the thyroid gland, radioisotopic destruction of the thyroid gland by the use of 131I, and surgical removal of the thyroid gland. After considering all the options, especially the possible effects of radiation on gamete development, Michael chose surgery. Following successful surgery, Michael was prescribed synthetic thyroid hormone to ensure that his body was receiving adequate thyroid hormone and was told to return within 2 months for a follow-up evaluation of circulating thyroid hormone concentrations. He was also cautioned to carefully monitor his calcium intake.

Using the the terms covered this week answer the following questions about this case:

Why would an imbalance in thyroid hormones have such a widespread effect on the body? Why was a goiter observed in Michael’s neck? Given that Michael and his wife may want to have more children, why was radioisotope, for the destruction of the thyroid gland, ruled out?

Needs Assessment

 When you are engaged in program planning, a needs assessment helps to determine the actuality and details of a specific problem. It also addresses how the target population views the problem—which is critical since their perspective and motivation to change are essential for the success of the program.
 

In this week’s video, the necessity of appraising a community’s assets as well as its challenges is evident as Dr. Donna Shambley-Ebron and Dr. Rebecca Lee discuss needs assessments. What additional insights have you gleaned from the Learning Resources that you intend to apply to your program?
 

For this Discussion, you develop a framework for completing a needs assessment for your program. To do this effectively, it is essential to begin with the program and the population you have identified and then develop an appropriate approach for conducting a needs assessment. This also leads to consideration of the types of data needed and the data collection methods to be used.

To prepare:

  • Consider      aspects of your problem and population (breast cancer in African American women      in the USA) as you review the information on needs assessment presented in      the Learning Resources.
  • How      would you conduct a needs assessment? What considerations would influence      your decision making about this process?
